I designed this immunization record printable because you know how overwhelming the schedule can become if you vaccinate your child.
It all becomes a blur: the many vaccine names, the date they should or have received, and any notes you want to remember about reactions or additional doses needed.
I dislike looking up information in my toddler's online chart; there are too many details to quickly find what I'm looking for.
I also prefer to go into each appointment knowing if she needs any shots that day. (I typically won't schedule anything after it so she can rest.)

HOW TO USE THE IMMUNIZATION RECORD PRINTABLE
The printable is designed minimally on purpose so that you can use it to suit your child's vaccination needs.
I recommend filling in your child's name in the top left corner.
The first column includes larger boxes to list all the vaccine names your child will or has received.
The top row offers smaller boxes to assign the ages your child will or has received each vaccine dose.
I would assign the last column as "notes" for a place to write down anything you want to remember about each immunization, like the brand name of the vaccine, reactions, or subsequent appointments.
When your child receives a vaccine, I recommend entering the date in the corresponding box in the center of the chart (instead of a check mark or "X").
The the titles are not pre-populated like most immunization record printables you'll find. I wanted to give you total control and freedom over how to fill out the record.
A RECAP OF HOW TO FILL OUT THE PRINTABLE:
- Write your child's name in the top left corner.
- List all their immunizations in the first column.
- Fill in the appropriate ages in the top row.
- Enter the date in the corresponding box whenever your child receives a vaccine.
- Add any notes in the last column about each shot received.
